You can customize the email from the “Email” settings menu of the plugin. Did you try going to the email settings interface?
I can’t find any occurrence of the spelling “Notificationservice” in the membership plugin. Are you sure that this is coming from this plugin?
We have created an addon to customize the registration and edit profile fields. Would you like to test it?
Please do your test with any other email address but the one you used and you will see how it works fine.
I am pretty sure you are testing with an email address that is already being used by a WP User account. Maybe your admin account is using that email address. Check the list in the WP Users menu and you will see that email address being used already.

Did you enable the following option from the settings menu of the plugin?
Enable Sandbox Testing
You should also check the “Enable Debug” field too.
