I have confirmed that the download package points to version 4.2.6, which is the version that I and others already have installed. Hope that helps.

Current Form Builder version is Version 4.2.6. Will remove and re-download and re-install, and report back. Thanks!

Just wanted to add I’m also having this issue. Nothing checked in Form Builder. In WP Membership > Settings > Email Settings I have “Send Notification to Admin” checked, and have entered custom text entered in the “Admin Notification Email Body.” The custom text I have is:

Here are the details:

Name: {first_name} {last_name}
Email: {email}
Username: {user_name}
Password: {password}

However, when a new member registers, admin only receives the “A new member has registered. The following email was sent to the member.” followed by the text specified in the “Email Body” section in WP Membership > Settings > Email Settings.
