It hasn’t really yet, I hope it gets updated and that it’ll fix the problem. I actually tried a different plugin now and it’s working good for me.

Hi Mbrsolution I did save the form this was about a week ago and at first it was on the list and all was working fine, the form was on the website the way I changed it. But now I want to change it and it’s all gone off the list, so yes I did try create a new form and save it again. It allowed me to create a new form a few times but it won’t go on the list any more. Now when I try I get an error :Error! Combination of membership level and form type must be unique,

The form is actually incorrect now on my website and the only way it changes is if I deactivate form builder- then it changes to the default form.
I tried to attach an image of my list hope you can see it.
