ParticipantI’m using formbuilder to create login forms and it works fine on the website,I wanted to change some of the fields but the form list is empty so their’s nothing to edit. I tried create a new form but if I use the same form name it says the form must be unique, so it seems it’s still there, but why does it not show in the list?
also when I try create a custom profile form I get an error message that I need to create a registration form first.
I tried uninstalling the formbuilder addon and reinstalling again but I get the same thing, I wish the least I could do is start form scratch again…May 8, 2015 at 1:15 pm #1721mbrsolution
ModeratorHi mikeberg when you created the form did you save the form? What happens if you create a new form just for testing purpose with only the default fields. Does the form appear in the list?

ParticipantHi Mbrsolution I did save the form this was about a week ago and at first it was on the list and all was working fine, the form was on the website the way I changed it. But now I want to change it and it’s all gone off the list, so yes I did try create a new form and save it again. It allowed me to create a new form a few times but it won’t go on the list any more. Now when I try I get an error :Error! Combination of membership level and form type must be unique,
The form is actually incorrect now on my website and the only way it changes is if I deactivate form builder- then it changes to the default form.
I tried to attach an image of my list hope you can see it.

ParticipantIt hasn’t really yet, I hope it gets updated and that it’ll fix the problem. I actually tried a different plugin now and it’s working good for me.
