New Member Record not created

I have multiple memberships for my site. I use Paypal hosted buttons using the advanced variables for subscriptions and buy now packages. I have had no issues with the subscription membership registrations. I just had a new member purchase a package using a buy now button, and it failed to create their member record in the system.

1) Why would it not have created them in the system?
2) Should I manually add them using the “Add Member” feature? Or is there a way to have them go through the process?

I am still trying to troubleshoot the issue. Since the first issue occurred, I have had two successful registrations using the same method. Then, I had one unsuccessful registration. It appears to be random.
Here is the log for the last two identical transactions using the same paypal button, the first working and the second not working.
