I can give you access to my site as well if it will help. I have the same problem as Jason – it’s hit-and-miss and I can’t solidly duplicate it all the time.
I have the latest version 2.1.6 already.

Yes, we continue to have major problems with it. Tried everything we can think of and spent many hours on it. We can’t seem to find any logic to the problems. Sometimes it works, sometimes it doesn’t, and we can’t figure out why it’s random. We even put two machines next to each other and got different results doing the exact same thing on both machines.
I thought that, too, but I have no WP cache enabled and I cleared cache on multiple machines.
I realized it started to happen after I installed the home page redirect plugin, so I disabled it and it started working for a while on one of my machines, but not the other one.
I’ve been playing with it all day and just can’t get it to work properly. I actually get different results on different computers (I have several computers).
When I first installed the plugin, I tested it and I didn’t have any problems. But sometime later (and after the redirect plugin) I have so many problems that the plugin just doesn’t work at all. Even after disabling the redirect plugin, it doesn’t work properly.
I have the same problem with not being able to log-out. I also have other problems when it says I’m logged-in, but I’m not. It sometimes says I’m not logged-in, but pages that are protected are still available. It seems random and there are many problems with this. I can’t seem to get it to work properly at all.
