This was the first time it happened. So I tell whether or not it is random.
I did have the the SWPM debug turned on. Every thing was successful. The first name, last name and email address of the new registrant were all correct in the log.

I also have MailChimp integration addon installed. I can see the wrong name (James Bihar) in the log:
