Upgrade membership level with a form

Hi,

The website I’m working on currently has 2 levels of membership, Free and Tier1.

In order to access Tier1 content, the user will have to be a member level of Free first. I understand that I can create a button to upgrade the member tier with the shortcode

[swpm_update_level_to level=”3″ button_text=”Update Level to Tier1″]

and hide it in a page that only Free members can access but I would like the user to fill in more profile fields before granting the member level to them. How do I create a form like this with simple membership?

Something like “Access more content by filling out your profile page”.
