Require Terms and Conditions on Registration (using WPSM Forms Builder)

The check mark and link do not show on the registration form
Using Form Builder to build the registration form with added fields
Using Pay then Register process.
Set Advanced setting and checked require Terms and conditions
Entered a valid url where Terms and conditions are located. (tested URL)
Result and Issue
Users get the link to complete registration and the registration form created by Forms builder is presented.
HOWEVER, the Agree to Terms and Conditions check mark and link does not show on the form

Hi, with the form builder, you can create any checkboxes (for terms and conditions) that you want to create (that gives you greater flexibility in terms of exactly what checkboxes you want to display and where you want to show it on the form). Simply add a checkbox with that checkbox being a “required” field. So the user has to agree to it before they can complete the registration.
