Need form builder (bought it in 2015) do I need to purchase again?

Hi guys, simple membership plugin is awesome.
Did the job fro me in 2015, when I installed it and bought the Form Builder for $40
I have the old license key.
Old email link to download it does not work.
Do I need to re-purchase Form builder again?

Form builder says “Reached maximum allowable domains” when I enter my old license.
If you check your records, the form builder was bought for a domain which exists and now just points/redirects to our new domain. It’s the same single business. I can pm you my license so you can verify.
Please help.
