Hi, can you check to make sure you are running the latest Form Builder addon version. You can download the latest version by clicking on the following URL re-download-the purchased add-ons.

I deactived form builder then reinstalled it. Then downloaded the zip file from the email link and activated that. Still getting the same error message. There is an html issue on line 54
