Form Builder Addon Database

Hi,

I have built a custom form for the membership site. I want to download this information into a report (I added a report builder, since this plugin does not have the capability to download the memberhsip information).

The basic form information is stored here: wp_dbww_swpm_members_tbl but the custom fields are not showing up. Does anyone know where I can field the custom fields, what database table?
