Form Builder Add-On Page does not have a link to download the plugin

I’ve tried searching the wordpress plugins directory and I cannot find the form builder plugin there. Plus there is no link on this page of the website: https://simple-membership-plugin.com/simple-membership-form-builder-addon/

Can somebody get me the link where I can download this plug-in or tell me specifically what to type in to the plugin directory search box?
