    I’m trying to use the formbuilder add-on, but I’m not able to do so.

    I get the error “Error! Combination of membership level and form type must be unique.” no matter what I do.

    To be clear, I don’t have any forms created yet, and none are listed on the page. This is getting a little frustrating, and I would appreciate your help.

    Any luck? I am getting the same error message and I have no forms created either.

    the error comes from code that references obsolete db fields. i have applied a fix. it should be ok now.

    Can you post a link so that I may be able to update the add on? Or share with us the fix.
