ParticipantI had this problem, and I was using the form builder add on. I deactivated the add on and went back to the original registration form and this cured the problem.
I was using the original free version of the form builder add on, I have noticed that the add on is no longer free, so maybe the old version no longer works.

I just installed the plugin and have been working with it. If you have the settings programmed for a “Pending” status, then when a person registers, they are placed in a “Pending” status until you approve them and update them to “Active”. At that time when you do that (This is located in the “Members” area of the plugin. You will see each person who has registered. Change their status to “Active” and place a checkmark in the “Notify User” box. This will open up the programmed email that will be sent as soon as you click on the “Edit User” button at the bottom which is actually the submit or save button.
The user already received the email confirming their registration form was submitted. These steps activate the “Completion” email to be sent. This is all being accomplished with the settings set for a “Free” membership and the user set as “Pending” so that it functions like the steps of the “Paid” options, but without any payment needed.

ParticipantI’m also getting this error when I use the Form Builder plugin with Simple Membership.
I can send emails manually via the Edit Member screen, but registration confirmation emails are NOT automatically generated and sent out when a user registers with the Form Builder plugin active.
When I disable the Form Builder plugin the automatic emails resume.
I like the Form Builder, but paying $30 for a plugin that disables core functionality of the original plugin isn’t acceptable.
If the error isn’t acknowledged or addressed in the next few days I’ll be getting my money back from PayPal.
