Can you please tell me where you clicked that gave the 404 error? Is it in the menu link that shows under the simple membership main admin menu?
Yes. All the files are in the right folder. It’s ‘Form Shortcode’.

The Registration form only allows Free membership. There are three levels: Free, Associate, Full. For the moment, all are free but users need to register.
Have you created new WordPress pages for each of your free member level registration and put the shortcode there? According to what the following usage documentation says?

This is the problem:
Step 1) After you install this addon, go to the “Form Shortcode” menu from the admin dashboard. Select a membership level from the dropdown list to generate the registration form shortcode for that level.
‘Form Shortcode’ returns a 404 error for …/wp-admin/swpm-form-shortcode
All files are present in the correct folder.

We have fixed the issue in the for shortcode addon. Please upgrade to v1.1 and this issue will be fixed.
Thanks – will do, and appreciate all the help. I got around the problem by copying the shortcode in the instructions for the addon.
