    I am attaching a capture that I would like members to be able to pay either per month or per year as presented here.

    I would also like the payment forms to be on the same page, is that possible?

    Hi,

    Judging from the URL above it looks like you want to have two different membership levels buttons in the same page. Is that correct?

    Yes exactly that he can pay per month or per year that the buttons are placed one below the other
