    Using a personal Paypal account doesn’t allow the Advanced Settings window so I can enter the code:

    notify_url=http://www.mydomain.com/fanpage/?swpm_process_ipn=1
    custom=subsc_ref=3

    Do I need to upgrade?

    It should be available to all PayPal account holders. You don’t need PayPal pro. Just the standard PayPal will do the job. You can try upgrading it to a premier account (it is free).
